The form shown was created using the Aweber form generator, and you can simply change the hidden field values to use it on your own site. Just find the commented code in the feature.php file and follow the instructions.

Easily insert a video in this space too. The default 920 pixel page width will accomodate a video up to 500 pixels wide along with the opt-in form shown.
